I use Everything 1.5a on two systems, Win7 and Win10 Pro. I have absolutely no issues on Win7, but the Win10 machine keeps losing the run counts.
I really have no idea where to start with this. Is this a known issue? Are there any fixes?

Please make sure Everything is installed correctly:
Are you using a third party tool that might be deleting your %APPDATA%\Everything\Run History.csv ?
- In Everything, from the Tools menu, click Options.
- Click the General tab on the left.
- Check Store settings and data in %APPDATA%\Everything.
- Click OK.

Thank you for the prompt reply! I have confirmed that the setting "Store settings and data in %APPDATA%\Everything." IS checked.
Also, I am not using any third party cleaning tools such as CCleaner which might be deleting this. Would using Microsoft One Drive for cloud backup be interfering somehow?

Might you be restoring old data - from the cloud, overtop newer data (on disk)?
Physically check %APPDATA%\Everything.
Verify that the .csv is dated current.
Verify that there are no incrementally created files; like Run History-1.csv, Run History-2.csv, Run History-3.csv ...

One Drive should be backing this file up TO the cloud, not over writing old versions FROM the cloud though. I'm not sure how I would check that to be the case though.
There are no incremental files as you mentioned. There is Run History-1.5a created today at 1pm, and Run History-1.5a.backup created on 7/13. It is a very small file, only 70 entries, which is bizarre because I have been using 1.5a on this machine since January.

Yes, I start most files from Everything. The fact there are only 70 items on the run list is extremely inaccurate.

Do a search, you can use a program named Everything for that, for & see what might turn up, perhaps elsewhere?
I've got 100K entries in mine (& virtually all are superfluous at that).
(BTW, should have looked first, & never should have tried to open it in my text editor [as it isn't made to handle some so big, 11 MB] .)

In that regard, does the fact that Everything displays "icons" ("thumbnails" or whatever it may be), does that then cause Everything to see that as a "Run" & hence a record is created of that "Run"?I've got 100K entries in mine (& virtually all are superfluous at that).

Maybe a wild guess... Do you have Tools -> Options -> History -> Keep run history for xx days enabled?
Does your system time change?
Everything saves run history to disk when you exit Everything.
Maybe Everything is not shutting down correctly..
Run something from Everything, exit Everything and see if your Run History updates:

- Run C:\Windows\System32\calc.exe
- Exit Everything (File -> Exit)
- Open your %APPDATA%\Everything\Run History.csv
- Check for an entry for calc.exe

Ahhh! That was it! I don't know how that got checked, but hopefully that clears up the problem. Makes sense why my run history was completely deleted after I went on vacation for a week Thank you!
