I am always filtering results and forget when I return to Everything that it is filtered.
Can an icon be put on the top menu bar to show results are filtered or not like in Excel auto-filters?

I will consider an icon on the top menu bar to show/clear the current filters.
Active filters are displayed in the status bar (on the bottom right).
You can right click the status bar to enable or disable filters quickly.
You can also double click an active filter in the status bar to disable it.
To open a new Everything window without remember the last filters:

- In "Everything", from the Tools menu, click Options.
- Click the Home tab.
- Change Match case to Disabled.
- Change Match whole word to Disabled.
- Change Match path to Disabled.
- Change Match diacritics to Disabled.
- Change Match regex to Disabled.
- Change Filter to Everything.
